The Non-Medical Case Manager is responsible for providing guidance and assistance in assessing medical, insurance, housing, social services, community, legal, financial and other services to the patients of the Peter Ho Memorial clinic. The Non-Medical Case Manager will also support the Medical Case Management team with completion of intakes, psychosocial assessments, development and re-evaluation of individualized client service plans and follow up on referrals as often needed. Non-MCMs also perform client advocacy & patient retention activities, treatment adherence counseling and termination or inactivation of cases. Maintains documentation in the EMR & Ryan White Grant reporting systems, ensure accurate data entry of service deliverables, educates and consults with service providers and consumers concerning HIV disease and the process of meeting consumers’ identified needs.
EDUCATION, EXPERIENCE, TRAINING
1. Minimum of one-two years’ experience working in case management, discharge planning or other health related field.
2. Bachelors of Arts, Social Work, psychology or other health care related field.
3. Ryan White Case Management Certificate obtained within 6 months preferred.
4. Experience and knowledge in basic to intermediate computer skills (Microsoft Office: Word, Excel, PowerPoints, email, etc.). Behavioral Violence Prevention Training within 3 months of hire and maintain current.
