The Discharge Care Manager participates as a member of The Wellness and Recovery Center team. This program provides behavioral health urgent care to adults in need of rapid support and intervention for an identified mental health concern. The goal of the program is to assist individuals in stabilizing acute symptoms through clinical support and medication management for a period of up to 30 days. The Discharge Care Manager will function as a member of this team and will assist with discharge planning, as well as addressing identified case management needs which support the individual’s recovery. More specifically, the Discharge Care Manager will assist with expediting referrals to the next level of care for individual, link the person with community resources such as shelter, transport, or food pantries, and will provide ongoing supportive contact with individuals who have stabilized but are awaiting linkage. The Discharge Care Manager will use an agency vehicle to provide community outreach as needed/identified by the treatment team. Care is provided under the direction of supervising staff and the clinical team.

EDUCATION, EXPERIENCE, TRAINING
1. Bachelor’s degree in a behavioral health science from an accredited institution.
2. 2 years’ experience in a mental health setting strongly encouraged.
3. Valid Driver’s License – as per policy requirement.
4. Defensive Driving Course – as per policy requirement – (may be taken after hire)
