The Patient Navigator will be responsible for coordinating service and identified patients through a complete health system by assisting with access issues, identifying resources, providing educational material, and developing relationships with service providers and community. The Patient Navigator coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care. The Patient Navigator is a consistent care coordinator throughout the continuum of care and assess the physical, psychological, and social needs of the patient enhancing patient outcomes, increasing satisfaction, and reducing cost of care. The Patient Navigator is responsible for community education and screening as well as monitoring community outreach and promoting survivorship.
EDUCATION, EXPERIENCE, TRAINING
1. Bachelor's degree or four (4) years of applicable cumulative job specific experience required.
2. Current BLS certificate upon hire and maintain current.
