The Ambulatory Clinical Pharmacist is responsible for providing patient care and advanced clinical pharmacy services in the ambulatory pharmacy space, including referral based HOPD pharmacist managed clinics, specialty pharmacy, and population health pharmacy services. This role involves collaborating with prescribers in the ambulatory setting and providing direct patient care managing medication therapy. The clinical pharmacy specialist ensures effective communication with prescribers regarding identified medication related problems and potential solutions and documents patient interactions across various electronic medical record (EMR) platforms.
The Specialty Clinical Pharmacist manages patients to be started or currently on specialty medications in collaboration with prescribers in outpatient clinics with a focus on Oncology and Rheumatology. Monitors medication therapy for significant drug-drug, drug-diet, or drug-laboratory test interactions. Intervenes in circumstances of significant interactions, therapeutic duplications, and other occasions when pharmacodynamics and pharmacokinetic expertise is warranted. Communicates potential problems and likely solutions to the prescriber. Documents patient interactions on multiple computer platforms. Educates patients and their families about specific medications. Assists in the training of new employees. May assist in the selection of technical staff. Position carries responsibility for other members of the healthcare team. This individual will often coordinate his/her work to meet the ever-changing needs of the department/hospital.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $115,107.20 to $166,712.00 on an annualized basis. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
