Overview

We are seeking an Outpatient Pharmacist, also referred to as an Acute Care Pharmacist or Hospital Pharmacist. Outpatient Pharmacist’s provide medications and counseling to patients who are being discharged from the hospital or receiving care through our hospital clinics. The Outpatient Pharmacist will focus on ensuring patients understand their prescriptions, manage long-term therapies, and transition safely from inpatient to home care. Their role emphasizes patient education, medication adherence, and coordination with providers and insurance to support effective treatment outside the hospital. This position is on a per diem basis, meaning PD/PRN as needed.

Responsibilities

Essential Duties and Responsibilities (includes, but not limited to): 

  • Dispense discharge and clinic prescriptions by reviewing physician orders, preparing medications, and ensuring patients leave the hospital with the correct therapies for home use 
  • Provide individualized patient counseling to explain medication purpose, dosing, side effects, and what to do if a dose is missed, with emphasis on safe use at home 
  • Conduct medication reviews to identify potential interactions, duplications, or inappropriate therapies before patients begin outpatient treatment 
  • Collaborate with physicians, nurses, and case managers to clarify medication orders and ensure a smooth transition of care from the hospital to outpatient settings 
  • Assist with insurance processing and prior authorizations to help patients access needed medications, including coordination with patient assistance programs when applicable 
  • Educate patients on managing chronic conditions such as diabetes, hypertension, asthma, and anticoagulation, reinforcing long-term adherence and monitoring needs 
  • Perform medication reconciliation at discharge by comparing inpatient and outpatient medication lists, resolving discrepancies, and documenting changes for continuity of care 
  • Ensure compliance with hospital, state, and federal regulations while maintaining accurate records, labeling, and storage of outpatient medications 

Qualifications

Before we go any further, we do have some deal-breakers. You must have:  
 
1. Current state Registered Pharmacist License.

 

Additional Qualifications That Are a Plus: 


1. Experience in hospital pharmacy operations preferred.
2. B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.
3. BLS certification and ACLS certification preferred
