Registered Nurse (RN) – Accountable Care Organization (ACO)

Location: Central Maine Medical Center, Lewiston, ME

Full-time | Days | 8 -Hour Shifts

 

The Registered Nurse (RN) provides safe, patient-centered care within the Medical Group / Accountable Care Organization (ACO) setting. Delivers direct patient care, supports care coordination across the continuum, and ensures efficient patient flow and population health outcomes in accordance with clinical standards and scope of practice.

 

Central Maine Healthcare (CMH), a member of Prime Healthcare Foundation, is an integrated healthcare delivery system serving approximately 400,000 residents across central, western, and midcoast Maine through a network of more than 40 primary and specialty care locations. The system includes Central Maine Medical Center  (CMMC) in Lewiston, a 250-bed, not-for-profit regional referral hospital that is home to the region's only Cancer Care Center, an orthopedic ambulatory surgery center, and the Central Maine Heart and Vascular Institute. CMMC also serves as the central Maine base for LifeFlight of Maine, the state's only medical helicopter service.

 

CMH also includes Bridgton Hospital and Rumford Hospital, two 25-bed critical access hospitals that provide essential care to Maine's rural communities.

Responsibilities

  • Provide direct patient care in an outpatient/clinic setting
  • Coordinate care plans with physicians and interdisciplinary teams
  • Monitor and communicate changes in patient condition, diagnostics, and symptomatology
  • Respond quickly and accurately to changes in condition or treatment response
  • Support chronic disease management and population health initiatives
  • Guide and support clinical staff while maintaining standards of care
  • Collaborate across departments to ensure quality care, continuity, and patient flow
  • Ensure adherence to regulations, policies, and clinical standards
  • Participate in performance improvement, quality initiatives, and staff education
  • Serve as a clinical resource to staff and team members
  • Maintain accurate documentation and ensure HIPAA compliance

Qualifications

1. Current state RN license.

2. Current BLS certificate upon hire and maintain current.

3. One year of professional nursing experience in a clinic setting, preferred.
