Responsibilities

The Certified Case Counselor – Lead (CCC-MCM) is a non-exempt, with no direct staff reporting role, responsible for overseeing the administrative needs of the case management team of the Peter Ho Memorial Center (PHMC). The CCC-MCM Lead plans and manages work distribution of the case management team. The responsibilities include training new staff and coordinating with the Department Manager for scheduling shifts and lunch breaks. The CCC-MCM Lead will also support the case management team with completion of intakes, psychosocial assessments, development, and re-evaluation of individualized client service plans and follow up on referrals as often as needed. Maintains documentation in EPIC, NJDOH system (CareWare), and Ryan White Grant reporting systems (CHAMPS), ensuring accurate & prompt data entry of service deliverables of the case management team. The CCC-MCM Lead educates and consults with service providers and consumers concerning HIV disease and the process of meeting consumers identified needs.

Qualifications

The lead will be responsible for all duties of CCCII to include providing direct case management services to patients of PHMC on an estimated ratio of 50% of Certified Case Counselor II (MCM) caseload. The CCC-MCM Lead provides guidance and assistance in assessing medical, insurance, housing, social services, community, legal, financial, and other services to the patients of the Peter Ho Memorial Center. CCC-MCM Lead responsibility also includes client-specific advocacy and review of use of services, including face-to-face, telephone, and other forms of communication.
 
EDUCATION, EXPERIENCE, TRAINING
 
1. Minimum of five (5) years’ experience. Working in case management, discharge planning or other health related field preferred. Working with persons with or at high risk of HIV infection preferred.
2. Bachelor Level degree required, Master’s in Social Work, psychology, or other health care related field preferred.
3. Prior supervisory experience is preferred.
4. Ryan White Case Management Certificate obtained within 6 months of hire required.
5. Experience and knowledge in basic to intermediate computer skills (Microsoft Office: Word, Excel, PowerPoints, email, etc.). MCM training within 3 months of hire and maintain current.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $17.33 to $30.53. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
