The Nurse Navigator will be responsible for coordinating service and identified patients through a complete health system by assisting with access issues, identifying resources, providing educational material and developing relationships with service providers and community. The Nurse Navigator coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care. The Nurse Navigator is a consistent care coordinator throughout the continuum of care and is able to assess the physical, psychological and social needs of the patient enhancing patient outcomes, increasing satisfaction and reducing cost of care. The Nurse Navigator is accountable for tumor cancer conference and ensures all standards are met for accreditation associated with cancer service types. The Nurse Navigator is responsible for community education and screening as well as monitoring community outreach and promoting survivorship.
Education and Work Experience
Employees must be in their current position for a minimum of six calendar months and not received any disciplinary action within the last six months to apply and be considered for a transfer.
