The Case Manager / Patient Navigator is responsible for all attributed patients with a primary Mental Health and/or Substance Use Disorder diagnosis with the overarching goal of achieving improved health outcomes through linkage to clinically appropriate treatment.
The specific role will be to ensure all QIP-NJ required screening is completed, clinically appropriate care and treatment is coordinated using evidenced based guidelines, patient is linked to hospital and community resources, and barriers to care are removed. The Case Manager / Patient Navigator will be part of a multidisciplinary team and will facilitate communication about patient needs with the team members. In addition, the Case Manager / Patient Navigator will promote self-management of mental health and/or substance use and provide psychosocial support as needed. The navigator will work in multiple hospital settings (inpatient, outpatient, ED) as well collaborate with external community partners to achieve patient and program objectives for the attributed population. The Case Manager / Patient Navigator will assist in the data management and reporting of QIP-NJ project initiatives.
