Provide education (appropriate to client), prompting and support to client and/or family designed to resolve crisis, and decrease barrier for more independent living. Assist in developing and facilitating service planning to promote further independence and community integration.
Supervise, engage and support adolescent consumers in a residential setting. Responsible to ensure medication monitoring and education, progress on Master Treatment Plan, personal care skill development, age appropriate group, leisure and recreational activities. To be able to communicate and collaborate effectively with families, program psychiatrist, and other mental health providers. Provide direct care, assisting adolescent clients with all activities of daily living, as needed using direction, modeling, and prompting.
