FOR INTEGRATED CASE MANAGEMENT, AND RESIDENTIAL:
Provides community-based mental health case management. Degree and scope of service intervention vary based on program as well as the changing needs of the individual.
FOR ADOLESCENT PARTIAL CARE, ADULT PARTIAL CARE, AND ACUTE PARTIAL HOSPITAL:
Responsible for providing direct care services including individual and group counseling, support and other activities;
participating in comprehensive assessment, development of treatment and discharge planning; maintaining clinical documentation,
participating in and facilitating family and collateral meetings, team meetings and case conferences to ensure continuity of care.
EDUCATION: Bachelor's in social work or psychology.
EXPERIENCE: A minimum of one-year relevant experience in a mental health setting. Individuals working with Adolescents must have at least one year of experience working with youth in a mental health setting.
TRAINING: Basic understanding of mentla health services and role of case managers n mental health services.
LICENSURE: Valid NJ Motor Vehicole License required.
