Care managers will assist person receiving outpatient services in adhering to treatment, encouraging responsibility for their treatment, maintaining follow-up appointments, scheduling and keeping appointments with other community medical providers, tracking of data and completion of agency documentation and problem solving with persons on potential barriers or issues that could impact recovery or progress.
Services provided under the directive of supervising staff.
EDUCATION, EXPERIENCE, TRAINING
1. Bachelor’s Degree in Behavioral Health Science from an accredited institution with 2 year’s post Bachelor’s
experience in the provision of mental health services.
2. Valid Driver’s License
