SOCIAL SERVICES II: To provide emotional support, crisis intervention and counseling for Pediatrics, Adolescents, Adults, Older Adults and their significant others to cope with a multiplicity of social and emotional problems; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability or trauma.  To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans and work as a member of a multidisciplinary team, and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.

• Responsible for direct data collection and data assessment of the patient’s condition and stability based on psychosocial assessments.
• Identifies biophysical, psychosocial environment, self-care, educational, spiritual and discharge needs of the patient as evidenced in the patient’s medical record.
• Responsible for the initial assessment and ongoing reassessment of the patient’s condition, stability and determination of psychosocial needs.
• Collaborates with other professional disciplines to plan, implement, evaluate and revise the Plan of Care for each patient, as evidenced in the patients’ medical record.
• The Plan of Care is discussed with and developed as a result of coordination with the patient/family/significant others when appropriate, is responsible to ensure appropriate signatures or documentation is in the medical record per policy and procedure.
• Develops Plan of Care using theories, methods and modalities consistent with education and training.
• Responsible for management of patient throughout stay, including discharge planning and effectively coordinates with others involved in the patient’s treatment and after care plan.
• Assesses and reports domestic violence, elder, dependent adult and child abuse , in accordance with regulatory standards and hospital policy and procedure.
• Bases decisions on facts, clinical reasoning and sound judgment; seeks assistance when necessary.
• Communicates with the Director or designee on all pertinent decisions related to behavioral contracts, possible discharges or transfers.
• Responsible for the evaluation of the patient’s response to/effectiveness of the implementation of the Plan of Care, communicates concerns during treatment team and commutates with others involved in care as appropriate
• Documents all significant contacts and services provided in the medical record.
• Co-facilitates education/group therapy with ancillary personnel.
• Facilitates process/education groups and completes requisite documentation.
• Provides Alcohol and Drug Screening and brief intervention, as appropriate, makes a referral to community resources if appropriate and patient is agreeable. Documents need for treatment and patient response in the medical record.
• Sets patient care priorities appropriately. Provides patient care activities within scope of practice and level competency and works cooperatively with others to equalize workloads of assigned personnel, as appropriate to the program functionality.
• Participates in clinical record review, chart audits and other support tasks, effectively and as appropriate.
• Notifies Manager/Charge Nurse/Director/House Supervisor, immediately of significant problems or unusual incidents and when appropriate, takes actions to intervene.
• Provides crisis intervention and supportive counseling to patients/families, as appropriate.
• Conducts support groups for patients/families as indicated.
• Develops and maintains community resource directory for all patient care areas.
• Assists in data collection and statistical analysis as needed.
• Attends and maintains mandatory training in safe patient handling, trained in safe lifting techniques, includes but not limited to 1) Appropriate use of lifting devices and equipment, 2) Five areas of body exposure: vertical, lateral, bariatric repositioning and ambulation, 3)Use of lifting devices to handle patients safely, as appropriate to program requirements.
• Performs all other duties as assigned or requested.

EDUCATION, EXPERIENCE, TRAINING

Required qualifications:

1. Master’s degree in Social Work, Marriage and Family Therapy, Licensed Professional Clinical Councilor or related field.
2. In states requiring license, required upon hire and maintain current in accordance with state regulations. In states without a licensure requirement, current registration for with appropriate state board required and/or under clinical supervision to meet clinical hours required for licensure.
3. Current BCLS (AHA) certificate upon hire and maintain current, required.
4. Behavioral Violence Prevention within 3 months of hire and maintain current annually.
5. Minimum one (1) year Inpatient l or Outpatient Behavioral Health experience. Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).
6. Receive/ maintain LPS Certification as deemed necessary.
