Participates in the coordination and implementation of the hospital Chemical Dependency Program, and to provide direct services to patients and their families. Performs treatment coordination duties for assigned patients in the Program. Responsibilities include regular liaison with members of the treatment team, discharge planning and aftercare follow-up and marketing with community agencies and professionals. The Chemical Dependency Counselor will document clinical assessments and treatment plans for assigned patients and participate in the Performance Improvement Program pursuant to specifications of monitoring devices.


  • Initiates and completes Chemical Dependency assessment within 72 hours of admission.
  • Evaluates Adult Unit patients for appropriateness to transfer to Chemical Dependency Unit within 24 hours of a physician’s order being written.
  • Works with RN to assure effective unit operations by prioritizing patient care, problem solving and fostering collaborative working relationships.
  • Collaborates with physicians and the interdisciplinary team daily to assist with planning, implementation, evaluation and revision of the plan of care for each Chemical Dependent patient.
  • Regularly schedules one on one contact with patients to assess/reassess needs related to substance abuse.
  • Documentation reflects contact with community agencies to coordinate treatment planning and aftercare planning when outside services are needed.
  • Collaborates with Social Worker in developing discharge plan and providing family sessions.
  • Maintains log documenting patient’s admission and detox vs. rehab status.
  • Actively participates in daily Case Management.
  • Documents/revises individual, discipline-specific treatment plan goals on patient’s Plan of Care prior to treatment team meetings.
  • Ensure that each patient is provided with an orientation to the unit and unit rules.
  • Documentation of groups reflects the progress toward treatment goals and objectives and interventions applied in the groups.
  • Conducts or co-facilitates didactic, support and educational groups per Chemical Dependency schedule.
  • Provides a minimum of two (2) in-services per year for staff.
  • Participates in treatment team meeting as noted by signature on patient’s Plan of Care.
  • Communicates consistently with psychiatrist and interdisciplinary treatment regarding treatment coordination.
  • Assists interdisciplinary treatment team with drug and alcohol related objectives and interventions for patients and families.
  • Problems in group are addressed in group therapy supervision or in individual supervision.
  • Bases decisions on facts, clinical reasoning and sound judgment; seeks assistance when necessary.




1.    AA/BA degree with Chemical Dependency experience preferred.

2.    Certification with State or National CADAC Board preferred.

3.    Minimum of two (2) years of clinical experience in psychiatric, chemical dependency or residential treatment facility preferred.

4.    Experience in counseling chemically dependent adults and their family members and/or significant others.

5.    Working knowledge of the 12-Step Program.

6.    Current BCLS (AHA) certificate upon hire and maintain current.

7.    Behavioral Violence Prevention within 6 months of hire and maintain current annually.

A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $17.95 to $25.24. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
