This Care Manager position is for the Enhanced Outpatient Program (EOP) located at 50 Morris Ave., Denville NJ.  EOP provides intensive individual and group therapy to adults diagnosed with a primary mental health concern or co-occurring diagnosis.  The primary modality for services is group therapy, and the average length of stay is approximately 6 months.  The Care Manager assists in coordinating referrals to the program, meets with new clients to provide orientation to services, and runs educational/supportive counseling groups.  The Care Manager also ascertains the case management needs of enrolled clients and assists with setting up referral/linkage to community resources as needed.  The Care Manager works as part of a multi-disciplinary team and interfaces with both internal and external providers.

Care managers will assist person receiving outpatient services in adhering to treatment, encouraging responsibility for their treatment, maintaining follow-up appointments, scheduling and keeping appointments with other community medical providers, tracking of data and completion of agency documentation and problem solving with persons on potential barriers or issues that could impact recovery or progress. Services provided under the directive of supervising staff.

Education and Work Experience

1. Bachelor's Degree in Behavioral Health Science from an accredited institution with 2 year's post Bachelor's experience in the provision of mental health services.
2. Valid Driver's License
