  • Ensures that all legal documents, such as LPS involuntary hold, psychotropic medication consent, conservatorship, etc., are promptly, accurately and completely filled out and appropriate advisement given to patient. Collaborate with other discipline in ensuring appropriate legal status of patients.
  • Conducts and co-facilitates didactic and support groups; and education groups as assigned.
  • Prepare and represent facility at Certification Hearings as needed.
  • Selects and implements psychiatric nursing interventions calculated to be most effective in the management of dysfunctional behavior patterns, e.g. hostile, acting-out, elopement, suicidal, depressed, psychotic, addictions, suspicious, confused, elated, ritualistic, and anxious.
  • Uses crisis intervention technique.
  • Facilitates the client to use his/her capabilities to his/her maximum potential.
  • Demonstrates knowledge of basic dynamics of group interaction.
  • Promotes a therapeutic milieu.
  • Recognizes the behavior patterns of consumer population in relation to multiculturalism.
  • Complies with and is able to fulfill the duties and responsibilities of department-specific Registered Nurse staff position when required.
  • Collaborates with and assists the Department Director on age-specific issues, clinical practice, unit operations, human resource management and/or regulatory requirements.
  • Serves as a resource to staff in area of clinical and managerial expertise.
  • Serves as a role model and provides positive influence to facilitate change, managing interpersonal concerns through chain of command in a professional manner.
  • Does not use unacceptable abbreviations in documentation, (uses read back on all telephone orders).
  • Follows medication reconciliation policy consistently and ensures compliance by all licensed staff.
  • Ensures Crash Cart, including defibrillation function, have been checked per protocol and after each code, ensures appropriate items are immediately replaced and crash cart checklist is complete with appropriate signatures.
  • Demonstrates knowledge of and compliance with information contained in
  • Infection Manual (i.e. OSHA blood borne pathogen standards, CDC Guideline), appropriate use of personal protective equipment (i.e., gloves, masks, goggles) and hand hygiene techniques.
  • Gives thorough, detailed efficient change of shift report to oncoming charge nurse to ensure consistency in the plan of care and assignment of nursing personnel. Uses the SBAR communication methodology in all reporting.
  • Assists physicians with special tests and procedures in the unit. Uses Universal
  • Protocol "time out" on all patients for any invasive bedside procedures and ensures staff compliance.
  • Avoids and prevents injury to self and patients by applying the principles of body mechanics and use of safe patient handling devices. Maintains a safe environment, functioning with an awareness and application of safety issues identified within the unit/facility.
  • Assesses and documents pain assessment and reassessment consistent with hospital policy and/or upon discharge or transfer of patients.
  • Recognizes signs of abuse and reports appropriately to Social Services and/or
  • Adult/Child Protective Services (APS/CPS) per county and state regulations.
  • Initiates discharge and patient / family education planning with proper documentation on the patient’s medical record.
  • Establishes priorities of patient care based on essential needs and available resources of time, equipment, personnel and supplies.
  • Formulates, maintains and updates a goal directed plan of care on patients which is prioritized and based on patient outcomes.
  • Reviews / implements new orders and keeps physician informed of patient’s status. Consistently follows telephone orders / read back policy. Does not use unacceptable abbreviations.
  • Accurately documents medications given to the patient on the Medication
  • Administration Record (MAR).
  • Fosters positive interpersonal relation with staff and members of other departments. Maintains high code of conduct and professionalism at all times.
  • Assesses, organize and coordinates patient care activities based on age appropriate interventions.
  • Assumes accountability for adverse events/consequences as a result of his/her independent judgment when assigning the staff and evaluating patient care outcomes.
  • Appropriately assigns, delegates, and supervises patient care activities utilizing independent judgment to assess the qualifications and experience/competency of the staff member and individual needs of the patient (acuity) and family.
  • Mentors staff by demonstrating empathy and respect for patients by explaining all procedures, providing privacy and allowing patients to express emotional needs.
  • Assumes leadership responsibility, assists in orienting new personnel, and assures that nursing care is carried out in accordance with departmental standards and physician orders.
  • Demonstrates ability for conflict resolution when resolving issues with employees and/or patients.
  • Consistently uses two patient identifiers when drawing lab, administering medications, blood components and performing procedures.
  • Documents evidence of nursing care in the patient’s medical record (EMR) based upon the nursing process in a legible, concise, timely and complete manner.
  • Utilizes the chain of command to ensure appropriate care of resolution of any issues pertaining to patient/staff well being or safety.
  • Participates actively in the scheduling of employees ensuring adequate coverage of all areas.
  • Makes frequent unit rounds to ensure safety, appropriate staffing and detecting any issues that require intervention. Resolves issues as able and requests assistance when needed.
  • Responsible in conjunction with the Director of the department to schedule staff for monthly and daily assignments. Properly adjusts and revises employee schedules to ensure all requirements of staff are assigned fairly among all staff members (weekend coverage, holidays, call and overtime). Assists with scheduling and monitoring master schedule for the Department staff. Monitors time and attendance for licensed and unlicensed staff.
  • Responsible for the observation and direction of patient lifts, mobilization, and shall participate as needed in patient handling.
  • Attends and maintains mandatory training in safe patient handling, trained in safe lifting techniques, includes but not limited to 1) Appropriate use of lifting devices and equipment, 2) Five areas of body exposure: vertical, lateral, bariatric repositioning and ambulation 3)Use of lifting devices to handle patients safely.
  • Uses professional judgment and clinical assessment in safe patient handling that are consistent with Safe Patient Handling policies.
  • Assumes leadership responsibilities by assisting with staff meetings, providing agenda items, educational opportunities, and items needing to be addressed for corrective actions. Attends and holds the meeting along with or in place of the Director when needed.
  • Supervises Patient Care Team to ensure that patients, families, visitors, physicians and other employees are treated with courtesy and respect.
  • Supervises and direct patient care in order to ensure timely assessment, treatment and transfer/discharge of patients in Behavioral Health.
  • Demonstrates effective problem solving skills in accordance with hospital policies and procedures.
  • Monitors staff resources to reflect the needs of the department. Has the authority to send staff home for illness, facility need, or performance issues in collaboration with Department Director/House supervisor.
  • Evaluates performance of staff in the Department through verbal and written counseling’s along with participation in performance evaluations.
  • Participates in interviews effectively and provides recommendation for hire to the Director for open positions
  • Performs other management duties requiring independent judgment as assigned by the Department Director, House Supervisor or CNO.
  • Performs all other duties as assigned or required.




1.    Current and valid state RN License

2.    Current BLS upon hire and maintain current.

3.    Previous supervisory experience, preferred.

4.    Behavioral Violence Prevention (BVP) certificate within 6 months of hire and maintain current.

5.    Working knowledge of Mental Health Laws, Practice Standards, interdisciplinary treatment team process, behavior health programming and daily nursing functions for inpatient and outpatient behavioral units.

6.    Continuum of care for behavioral health care, preferred.

7.    Receive/ maintain LPS Certification as deemed necessary.

8.    Must obtain approved training on LPS statuses and patient rights and successfully pass LPS Certification exam.

9.    Must have approved LPS certification prior to writing 5150 holds in designated Prime facilities.

10.  LPS Professionals: Must have experience in an inpatient mental health facility per County requirements. 

11.  Patients Rights Education/ Competency required 30 days upon hire and maintain current.

12.  Bachelor of Science in Nursing degree, preferred.
