The Nurse Practitioner focuses on providing general care to patients of all ages, with utmost patience, care, compassion and understanding. Perform tasks within their scope of practice which may include conducting tests, recording medical histories, providing treatments, assisting physicians and enhancing public awareness on the importance of health, and prescribing medications.
The Certified Case Counselor II - MCM will be responsible for providing direct case management services to the patients of the Peter Ho Memorial Center; Client-centered services linking clients with health care, psychosocial, and other services provided by trained professional, including both medically credentialled and other health care staff. The Certified Case Counselor II will support the clinical team with resources, information, and performs the various steps of medical case management including completion of intakes, psychosocial assessments, development of initial individualized client service plans, conduct client advocacy & retention activities, referral follow ups, re-evaluation of the patients’ needs/discharge, treatment adherence counseling and proper disposition of cases. Maintains documentation in the EPIC, NJDOH system (CareWare), and Ryan White Grant reporting systems (CAHMPS), ensure accurate & prompt data entry of service deliverables, educates, and consults with service providers and consumers concerning HIV disease and the process of meeting consumers’ identified needs. Also included is treatment adherence counseling to ensure readiness for, and adherence to complex HIV/AIDS treatments. Medical case management also includes client-specific advocacy and review of use of services, including face-to-face, telephone, and other forms of communication.
The Certified Case Counselor – Supervisor (CCC-MCM) is responsible for overseeing the administrative needs of hospitals or health care facilities, specifically the case management team of the Peter Ho Memorial Clinic. The CCC-MCM plans schedules and manage the staff, supplies, and work distribution of the case management team. The responsibilities include screening/interviewing prospective staff, training new staff, and scheduling their shifts. The CCC-MCM provides guidance and assistance in assessing medical, insurance, housing, social services, community, legal, financial, and other services to the patients of the Peter Ho Memorial clinic. The CCC-MCM will also support the case management team with completion of intakes, psychosocial assessments, development, and re-evaluation of individualized client service plans and follow up on referrals as often needed. Maintains documentation in the EPIC, NJDOH system (CareWare), and Ryan White Grant reporting systems (CHAMPS), ensure accurate & prompt data entry of service deliverables of the case management team. The CCC-MCM educates and consults with service providers and consumers concerning HIV disease and the process of meeting consumers’ identified needs. Responsible for assisting physicians with patient care, clerical, environmental and organization tasks. Responsible for appointment scheduling and chart management. Provides Information to patients so they may fully utilize and benefit from the clinic services. Conveys a positive image of organization.
