SOCIAL SERVICES II: To provide emotional support, crisis intervention and counseling for Pediatrics, Adolescents, Adults, Older Adults and their significant others to cope with a multiplicity of social and emotional problems; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability or trauma. To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans and work as a member of a multidisciplinary team, and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.

The Clinical Educator provides educational services for areas of clinical expertise and patient care services to attain the Hospital’s objectives, mission and values established by corporate administration. Supports corporate-wide programs, policies and procedures are developed to assess, evaluate and meet the needs of the patient. Assists with development and implementation of education plans for providing training in specialized clinical settings. The Clinical Educator provides hands on training for clinical staff to support the educational goals for the hospital. Implements an effective, ongoing training program to measure, assess and improve the quality of care, treatment and services delivered to all patients. The Clinical Educator shall work collaboratively with the other Prime Healthcare facilities to provide educational services to staff in the Perioperative Services Department.   The Perioperative Services Regional Educator will maintain knowledge and expertise in a specific area of nursing practice and will be responsible for providing education in this area of expertise under direction of the Prime facilities. The Perioperative Services Regional Educator will be responsible for providing education in person and/or virtually to support other Prime Healthcare facilities in their area of expertise. The Emergency Department (ED) Charge Nurse is responsible and accountable to the Department Director for the quality of nursing staff and the delivery of patient care during their shift and assists the Director in the daily operation of the respective unit(s)/department. The CNA in the Department is an certified assistive person who assists with activities of daily living, provides basic nursing care to non-critical patients and assists in maintenance of a safe environment under the direction and supervision of the Registered Nurse in charge of the team and/or unit.

The Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Provides direct patient care within the scope of practice. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of care in the ICU. The Registered Nurse is directly responsible and accountable for the care given to his/her assigned patients. They communicate with the physician about changes in the patient’s clinical condition including hemodynamic monitoring, results of diagnostic studies and abnormal results and symptomatology. Is able to respond quickly and accurately to changes in condition and/or response to treatment. Maintains patient privacy and confidentiality.

To provide clinical oversight and supervision to ASWs and MFTIs.  Assist Director in quality assurance and program development tasks for social services. To provide emotional support, crisis intervention and counseling; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability or trauma in all clinical care areas. To complete psychosocial assessments and work as a member of a multidisciplinary team. Complete necessary documentation and maintain clinical record for all social service consults during his/her length of stay.

The LVN/LPN provides direct and indirect patient care services requiring clinical competence within the scope of practice in accordance with state requirements. In collaboration with the healthcare team, under the supervision of a Registered Nurse (RN), the LVN/LPN works to maintain safety, provide patient comfort and care and improve the overall patient experience in the medical surgical setting.

Registered Nurse is responsible for the delivery of safe patient care utilizing the nursing process of assessment, planning, implementation and evaluation. The Registered Nurse also directs and guides patient/family teaching and activities of other nursing personnel while maintaining standards of professional nursing. Is able to respond quickly and accurately to changes in condition and/or response to treatment. Provides for a safe environment for patients and visitors. Utilizes the chain of command when necessary to ensure patient well-being and appropriateness of care to neonates from post delivery stabilization through discharge. NICU- Provides direct patient care within the scope of practice with assistance to neonates.

The LVN provides direct and indirect patient care services requiring clinical competence within the scope of practice in accordance with state requirements. - In collaboration with the healthcare team, under the supervision of a Registered Nurse (RN), the LVN works to maintain safety, provide patient comfort and care and improve the overall patient experience in the medical surgical setting. An ideal candidate will be able to work 1 - 2 shifts per week, or more and has at least 1 year of Acute Care Experience. Great way to earn an extra income while making a positive impact! This position qualifies for the + $1.50 per hour shift differential.

An ideal candidate will be able to work 1 - 2 shifts per week, or more and has at least 1 year of Acute Care Experience. Great way to earn an extra income while making a positive impact! This position qualifies for the + $1.50 per hour shift differential.

An ideal candidate will be able to work 1 - 2 shifts per week, or more! Great way to earn an extra income while making a positive impact! This position qualifies for the + $4.25 per hour shift differential.
