In collaboration with the manager/director of Performance Improvement, the coordinator is responsible for assisting in the coordination, integration, and implementation of the organization wide performance improvement activities to ensure compliance with company policies and state/federal regulatory and accreditation standards. Activities include but are not limited to chart reviews, data abstraction for CMS Core Measures and other quality outcomes studies, and staff education regarding core measures and updates regarding new and enhance indicators.
The Transition to Practice Educator provides continued educational support, training and mentoring to new nurses in the clinical area. Ensuring a smooth transition to practice by providing unit-based support, coordinating the needs and resources for each new nurse by follow up during and after the orientation period. The Transition to Practice Educator will serve as a clinical liaison between education and clinical managers/coordinators to facilitate a timely release from orientation by assessing needs and performance in live time.
